7. If self-employed, fill out the business name, address, contact information, type of business, and years of operation. You also need to outline your career plans and proposed research areas on the attached sheet.
8. For Section V, list your involvement in research and development over the last five years, including the field and title of your research, location/duration, fund source, and nature of involvement.
9. In Section VI, provide your publications from the last five years, detailing the title of the article or publication, place/year of publication, nature of involvement, and award-giving body.
10. Complete Section VII by listing any awards you have received. After filling out all sections, ensure you certify that all information provided is accurate and sign the document.
11. Finally, review your application and the checklist of required documents. Save your changes, then download, print, or share the completed form as needed.
