6. Select your subscriber scheme preference. Indicate your choice of Pension Fund Manager (PFM) strictly from the provided list and choose between active choice or auto choice for your investment option.
7. If you opted for active choice, indicate your asset allocation among Equity, Corporate Bonds, and Government Securities, ensuring the total equals 100%. If opting for auto choice, skip this allocation section.
8. Review and sign the Declaration and Authorization section. A clear signature or thumb impression is required, along with the date of signing.
9. Once all sections are completed, ensure you have filled in any necessary details for the POP-SP, such as signatures and official seals.
10. Finally, save your changes. You can download, print, or share the completed form as needed.
