- Fax the completed form to the RMV at 857-368-0819. Note that a call to the RMV is unnecessary as their fax machine is monitored regularly.
- Process an inquiry transaction within a half hour to ensure that the plate has been canceled. The RMV will send the Plate Return Receipt to the original owner.
- Select the Reassignment option in your EVR software to complete this transaction, understanding that you do not need to add the plate to the EVR plate inventory.
- Finalize the transaction and make a copy of the form for your files, ensuring that it is added to the customer folder.
- Attach the original reassignment form to the original RMV-1 and submit it to the RMV. Remember that UMS or EVR plate cancellation capabilities cannot be used for plate reassignments.
