- Enter your preferred name for the diploma and fill in a pronunciation guide for your name to ensure proper pronunciation at events.
- Provide basic personal information such as gender and date of birth, ensuring that this is for verification purposes only, and optional language capabilities beyond English.
- If applicable, state any relations you have to existing CAS members, explaining your connection.
- Identify and rank five job responsibilities that you perform in your current role using a scale from 1 to 5.
- Optionally, select your ethnicity from the list provided, if you wish to disclose this information.
