- Total the wages paid over the unemployment insurance taxable wage base for each worker and enter these amounts in Line 17.
- Subtract Line 17 from Line 16b and record the result on Line 18, which represents your taxable wages.
- Calculate your unemployment insurance contribution due for each quarter in Lines 19 and 20 according to the provisions provided.
- Enter the quarter totals on Line 21 and sum these totals for your grand total on Line 22.
- Provide the number of employees covered for unemployment insurance in Step 4, completing the appropriate lines for each quarter.
- In Step 5, document your total unemployment insurance contribution due on Line 27, and record any previous payments on Line 28.
- Subtract Line 28 from Line 27 to find your contribution due.
- If you have stopped employing workers, write the date you last employed them in Step 6.
- Finally, sign the form in Step 7, stating the report's accuracy and completeness, and ensure to provide your daytime telephone number.
