- State whether your research endeavor involves human subjects and ensure you have the necessary approval from the Human Subjects Committee if applicable.
- Confirm your willingness to observe and consult with faculty regarding at least four different courses, documenting them as required.
- Read and agree to the USET Student Agreement, signing where indicated.
- If applicable for your project, confirm the agreements discussed with your faculty sponsor, ensuring their support.
- Review the letters of recommendation waiver and decide whether to waive your right to read the letters sent on your behalf.
- Finally, save your changes, download or print the completed application form, and submit it via email to the appropriate address.
