Get Participant Information Form (one Form Per Attendee)

Completing the Participant Information Form is an important step in registering for the Asilomar Chromatin and Chromosomes Conference. This guide provides you with clear and supportive instructions to ensure you fill out the form accurately and efficiently.

Follow the steps to successfully complete your Participant Information Form.

1. Press the ‘Get Form’ button to access the Participant Information Form and open it for completion.
2. Enter your first name in the designated field.
3. Fill in your last name in the corresponding field.
4. Provide the name of your institution in the institution field.
5. In the address section, begin with Address Line 1. If applicable, complete Address Line 2 and Address Line 3 for your full address.
6. Input your phone number in the phone field.
7. Provide your fax number in the fax field, if relevant.
8. Enter your email address in the email field.
9. Select your status from the options provided: PI, postdoctoral fellow, student, or other.
10. If applicable, include the name of your PI in the designated field.
11. Specify the title of your oral presentation in the title field.
12. Answer the three questions to assist with registration tracking: 1. Date of payment for the Meeting Registration Fee, 2. Amount of the Registration Fee paid, 3. Indicate if you wish to order a BOX LUNCH for Sunday, December 16 (respond with YES or NO).
13. Review all the information entered to ensure accuracy.
14. Once all fields are completed, save your changes, then download, print, or share the completed form as needed. Email the form to goldman@sfsu.edu with ASILOMAR in the subject line by October 14, 2012.
