Follow the steps to successfully complete your Schedule CT-40.

1. Click the ‘Get Form’ button to access the Schedule CT-40 and open it for editing.
2. In the field for your name(s), enter the information as shown on your Form IT-40.
3. In the Social Security Number field, input your Social Security Number accurately.
4. On line 1, enter the amount from Form IT-40, line 7. If you and your partner lived in the same county on January 1, enter the full amount from line 7; do not complete Column B.
5. Refer to the county tax rate chart found on the back of the Schedule CT-40 and enter the tax rate for the county you lived in on January 1, 2024, in line 2.
6. In Column A for ‘yours’, multiply the amount from line 1A by the county tax rate you entered in line 2. Leave blank if the result is less than zero.
7. Complete lines 3A and 3B, where applicable, and add them together. For Perry County residents, complete lines 5 and 6 if you worked in specific Kentucky counties. Otherwise, report the total on line 7.
8. For those required to fill in lines 5 and 6, enter the amount of income taxed by the specified Kentucky localities in line 5 and multiply this amount by the Perry County rate in line 6.
9. Subtract any applicable amounts as instructed, and ensure the final total is accurate and reflects on line 7.
10. Once all information is complete, save your changes, download, print, or share the Schedule CT-40 as needed.
