The payer must provide you with a Form W-2G if you win: $600 or more if the amount is at least 300 times the wager (the payer has the option to reduce the winnings by the wager) $1,200 or more (not reduced by wager) in winnings from bingo or slot machines. $1,500 or more in winnings (reduced by wager) from keno.

File Form W-2G, Certain Gambling Winnings, to report gambling winnings and any federal income tax withheld on those winnings. The requirements for reporting and withholding depend on the type of gambling, the amount of the gambling winnings, and generally the ratio of the winnings to the wager.

Generally, if you receive $600 or more in gambling winnings, the payer is required to issue you a Form W-2G. If you have won more than $5,000, the payer may be required to withhold 28% of the proceeds for Federal income tax.

Gambling facilities are required to document your winnings with Form W-2G if they meet these thresholds: $1,200 or more in winnings from bingo or slot machines. $1,500 or more from keno. $5,000 or more from poker tournaments.
