5. Finally, complete the 'Payment' section. Enter your credit card information if applicable and indicate the card type. If you are paying by check, make it payable to BYU Independent Study. Your cardholder's signature is necessary to authorize the charge.
6. Review all information for accuracy. Once satisfied, you can save your changes, download the completed form, print it, or share it as needed.
