8. If applicable, indicate whether you have a disability. If you select 'Yes', you will need to specify your condition and the type of assessment received.
9. Fill out your education background, specifying your highest completed school level and any qualifications obtained, along with their completion dates.
10. Provide information regarding your employment status and the main reason for undertaking the course.
11. Review the Privacy Note, which outlines how your personal information will be handled. Acknowledge your understanding by signing the form.
12. In the Student Enrolment Declaration section, confirm the truthfulness of the information provided, grant permissions for data sharing as necessary, and sign the form.
13. After filling out all sections, ensure that you save your changes, and make a backup of the document. You may also choose to download, print, or share your completed form.
