Follow the steps to fill out the CC-40 form successfully.

- Enter the name(s) as shown on your Form IT-40/IT-40PNR/IT-41 in the designated field.
- Input your Social Security number in the appropriate box.
- Provide the name as shown on your IT-20/IT-20NP in the relevant section.
- If applicable, include your Federal ID Number.
- In Part I, list the eligible Indiana college or university in the first column along with the date of contribution in the format MM/DD/YYYY in the second column.
- Enter the 4-digit code number corresponding to the institution in the following column.

- Select the type of return you are filing from the provided options: individual, corporate, or fiduciary.
- In Part II, calculate the total contributions you made and insert the total in line 1.
- In line 2, enter 50% of the total contributions reported in line 1.
- Identify the limitation for your filing type (either $100 for single or fiduciary returns, or $200 for jointly filed returns) in line 3.
- Determine the lesser value between line 2 and line 3 and enter that value in line 4.
- In line 5, input your Indiana adjusted gross income tax from the appropriate line on the IT forms.
- Proceed to Part III for corporations. Enter the total contributions and follow the computation steps for credit as outlined.
- Finally, review all entries for accuracy. Save changes, download, print, or share the form as necessary.
