1. Click ‘Get Form’ button to access the checklist. This action will enable you to open the document for completion.
2. In the first section, you will need to indicate if you are being rehired or have completed the online application process for a new position. Print your first and last name in the designated space, then specify your department and the name of your position.
3. Provide your Indiana Tech email address in the space allocated. Notifications regarding the readiness of your Orange Card will be sent to this email.
4. Check with the Business Office to confirm your clearance status. If you are not cleared, you will not receive your Orange Card.
5. Ensure you have a current Form W4 (2010) – Federal Employee Withholding Allowance Certificate or confirm that your information is updated in the ADP Portal.
6. Confirm that you possess a current State of Indiana Employee Withholding Exemption and County Status Certificate. If necessary, attach the updated Indiana Tax Form.
7. Make sure you have a current Direct Deposit Authorization Form or that your information has been updated in the ADP Portal.
8. Submit this checklist along with any new or updated forms to the Career Center located in Andorfer 229.
9. Once you have submitted all paperwork, you will receive an email notification when your Orange Card is ready. Make sure to return to the Career Center to pick up your official Orange Card.
