- Complete Section C with card settings. Specify the sponsor department, default FOAP, campus address, approver/authorizer information, and account manager along with their details. Indicate transaction and monthly limits as needed by selecting either the default limits or specifying different amounts.
- Upon finishing the sections, ensure to sign and date the application in the approval/authorization section. Obtain the necessary signatures from your approver.
- After completing the form, print and sign both the Cardholder Agreement Form and the Sponsor/Authorizer Agreement Form as required.
- Submit the original completed application along with the signed agreements to Financial Services either in person or via inter-campus mail to PRK 252. If you need the card immediately, be prepared to pay a $25 rush fee.
- If training is required, watch for a confirmation email with information from Financial Services. Complete the training and notify them once done.
- Finally, once the card arrives, you will be notified by Financial Services about its availability for pick-up.
