Follow the steps to successfully complete your pledge form.

1. Click the ‘Get Form’ button to obtain the NC SECC Retiree Pledge Form. This will allow you to access and fill out the form online.
2. Begin by entering your title (e.g., Miss, Ms., Mrs., Mr., Dr.) and name in the designated fields. Include your first name, middle initial, and last name.
3. Provide your department or university details, along with the division or academic unit you belong to.
4. Fill in your home address, including the city, state, and zip code. Note that this information is optional but may be required for acknowledgment if you do not provide an email address.
5. Enter your daytime phone number and email address for further correspondence.
6. In the 'My SECC Pledge' section, select the causes you wish to support. Indicate whether you will use a credit card to pay online by providing the transaction number after making a payment at ncsecc.org/donate.
7. If you prefer to provide payment information directly, choose between one-time, quarterly, or monthly charges. Indicate your preferred credit card type and enter the required details, including card number, CVV code, and expiration date.
8. Specify your total pledge amount and indicate if you will be donating via check, cash, stock transfer, or a matching gift.
9. Designate your gifts by selecting the charities you want to support from the approved SECC charities list. A minimum donation of $10 per charity is required.
10. If you wish to keep your gift anonymous to the selected charities, be sure to check the appropriate box.
11. Review your entries for accuracy. Once completed, save any changes made, and you can choose to download, print, or share the form as needed.
