- Part II focuses on elements to be completed by the claimant. First, complete items 12 and 13 regarding the disposition of remains and the date of interment.
- Next, complete either item 14, 15, or 16, depending on what costs are applicable. Ensure you only fill out one of these items.
- If you are claiming shipping costs for remains, fill out item 17 and provide the relevant details including shipping origin and destination as well as shipping mode.
- Finally, in item 18, affirm that you have incurred the expenses listed, providing your name, address, taxpayer ID number or SSN, and ensure your signature and date signed are included.
