- If you wish to enhance your skills, check the box indicating your interest in the optional Mindleaders courses.
- Have your supervisor complete the Supervisor Statement of Approval, including their signature, title, date, and a brief statement regarding your candidacy.
- Fill out the Applicant Statement of Commitment with your signature, printed name, date, and title, ensuring all provided information is legible.
- After completing the application, save your changes and consider downloading, printing, or sharing your application form as necessary.
