The Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) is frequently used by clinicians when assessing individuals for obsessive-compulsive disorder. Its focus on symptom severity provides valuable insight into the impact OCD symptoms have on patients' lives.

Instructions: Generate a Target Symptoms List from the attached Y-BOCS Symptom Checklist by asking the patient about specific obsessions and compulsions. Chock all that apply. Distinguish between current and past symptoms. Mark principal symptoms with a "p".

In sum, 11 symptom areas—(A) contamination/cleanliness, (B) harming obsessions, (C) sexual obsessions, (D) checking, (E) religious/magical thoughts/superstition, (F) morality and justice, (G) symmetry/exactness/ordering, (H) just right/repeating rituals/counting, (I) hoarding and saving, (J) somatic obsessions and (K) ...

The Y-BOCS score is a determining criterion for inclusion in or exclusion from the protocols. A score equal to or greater than 16 is generally required for trials in medical treatment or psychotherapy, and a score of 25 or more is required for deep brain stimulation.

The Y-BOCS, a 10-item, clinician-administered scale, has become the most widely used rating scale for OCD. The Y-BOCS is designed to rate symptom severity, not to establish a diagnosis.

A diagnosis of OCD requires the presence of obsessional thoughts and/or compulsions that are time-consuming (more than one hour a day), cause significant distress, and impair work or social functioning. OCD affects 2-3% of people in the United States, and among adults, slightly more women than men are affected.

This scale is designed to rate the severity of obsessive and compulsive symptoms in children and adolescents, ages 6 to 17 years. It can be administered by a clinican or trained interviewer in a semi-structured fashion.

0 = None. 1 = Mild, slight interference with social or occupational activities, but overall performance not impaired. 2 = Moderate, definite interference with social or occupational performance, but still manageable. 3 = Severe, causes substantial impairment in social or occupational performance.
