8. For Section D – Record of Service in the Armed Forces and Training Organizations, provide details about your service, including the country, unit, dates, rank held, and relevant employment or courses.
9. In Section E – Civil Employment, document your employers' names and addresses, along with the dates of employment, positions held, and reasons for leaving.
10. Use Section F to cover any periods not included in previous sections, explaining how you were employed during those times.
11. Complete Part II and have it verified by a qualified individual as specified, ensuring they sign and date the certification of your birth certificate.
12. In Part III, obtain a certificate of character signed by a responsible senior person who has known you for the last five years.
13. If applicable, complete Part IV, providing a parental or guardian undertaking if you are under 18 years of age. Ensure all signatures and dates are correctly recorded.
14. Finally, review the completed form for accuracy, ensuring all areas are filled. Save your changes, and download, print, or share the completed form as necessary.
