An affidavit (/ˌæfɪˈdeɪvɪt/ AF-ih-DAY-vit; Medieval Latin for "he has declared under oath") is a written statement voluntarily made by an affiant or deponent under an oath or affirmation which is administered by a person who is authorized to do so by law.

This form is required whenever a property owner grants authority to another individual to sign on their behalf when obtaining a permit. It must be completed and returned prior to permit issuance.

Usage: An Affidavit for Authorization is a legal document used to grant authority or permission to an individual or entity to act on behalf of another person or organization. It is used in various situations, including financial transactions, legal matters, healthcare decisions, and property management.

An affiant is an individual who makes an affidavit. An affidavit is signed by both the notary public and the affiant, generally under witness. A statement of fact or a “sworn statement” is true to the best of the affiant's knowledge.
