- Begin with the Employee Information Form. Fill in all required fields with accurate information about the employee.
- Ensure the employer and employee sign the form where indicated. Once completed, make a copy for your records.
- Proceed to the IRS Form W-4. Fill in lines 1-7 with the employee's information accurately. Note that the employee may select either allowances on line 5 or EXEMPT on line 7, but not both.
- The employee must sign and date the form directly below line 7. Include the employer's name and CDC+ Consumer ID # in line 8 while leaving lines 9-10 blank. Remember to keep a copy for your files.
- Next, complete the DHS Form I-9. In Section 1, the employee must fill in all required information, check their citizenship status, and sign and date the section.
- In the margin at the bottom, print the last name of the consumer and their CDC+ ID #. Leave Section 3 blank and make a copy for your records.
- For Section 2, as the employer or CDC+ representative, verify the employee's credentials by examining the necessary documents. Provide the relevant information accordingly.
- Complete the Direct Deposit / rapid! PayCard Visa Payroll Card Request Form by following the printed instructions. The employee should select one pay option and attach a voided check for Direct Deposit.
- After filling out all forms, organize them in the specified order, along with the updated Purchasing Plan or Quick Update form, and submit to your CDC+ consultant.
- Finally, save your changes, and if needed, download, print, or share the completed forms for your records.
