Get Fidelity 529 College Savings Plan Distribution Request Form 2023-2025

Go to Fidelity.com/college or call 800-544-1914. Use this form to initiate a rollover or deposit of your 529 College Savings Plan, Coverdell education savings account, a qualified U.S. Savings Bond, or another 529 College Savings Plan (“529 plan”) to a 529 plan at Fidelity.

You can call your plan administrator, make a request online, or submit a withdrawal request form. The plan can send withdrawals by check to the account owner, the beneficiary, or the school. You can transfer the money to yourself or the beneficiary electronically and then make payment to the school.

Go to Fidelity.com/college or call 800-544-1914. Use this form to make a one-time withdrawal (either full or partial) from a 529 College Savings Plan. Do NOT use this form to transfer assets to another beneficiary. Type on screen or print out and fill in using CAPITAL letters and black ink.

Choose your 529 account from the drop-down menu and click "Add a Payee"

The SECURE Act only allows up to $10,000 in a 529 to be used for student loan repayment. This is a lifetime limit that applies to each beneficiary. For example, a family with two children can use $10,000 per child to repay their student loan debt.
