Follow the steps to fill out the membership form online.

1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Enter the parent name(s) in the designated field. Ensure that you include the full names of all guardians or adults who are members of this family.
3. Fill in the address. This should be the primary address where communication regarding the PTO will be sent.
4. Provide a contact phone number in the appropriate field. Make sure this number is one that you regularly check for communications from the PTO.
5. List the name(s) of your student(s) along with their corresponding grade and teacher. This section allows the PTO to keep a record of which students are associated with the membership.
6. Review the membership dues. The cost is $6 annually per family. If opting to pay by cash, ensure that you print and return the completed form by the designated due date.
7. If you are paying online, follow the provided instructions for payment to complete your membership.
8. Once you have filled out all required fields and provided payment information, you can save changes to the form. Depending on your needs, you can also download, print, or share the completed form.
