This form should be completed by a parent or permanent legal guardian (“Proxy”) who wants to request access to another patient's (or your child's) My CS-Link Record. The Proxy will need to show a photo ID and/or relevant legal or court documents asserting their right to make such request.

Proxy access enables, you (the Representative) to view and manage your family member's (the Patient's) medical records within MyChart. Depending on the access level granted, designated proxies can view information such as upcoming appointments, messages, test results and more.

Proxy access allows you to securely communicate with the patient's physician's office on behalf of a family member or person under your care.

Proxy Access allows a student to authorize other people to have view access to a student's 1098-T tax information. For example, a student could designate a parent or guardian as a proxy. The proxy, as an authorized viewer, would be able to see the student's 1098-T.

Patients with a MyChart account may allow another person (such as a parent or legal guardian) to access their MyChart account. This is called proxy access.

To request to use MyUnityPoint on behalf of someone else, log in to your MyUnityPoint account and select Share My Record on the drop down menu. Select Friends & Family Access to enter appropriate proxy request. Pediatric (Ages 0-11 Years) • Full access to approved parent/legal guardian(s).

From 11–16, a parent with proxy access will be able to manage certain elements of the young person's record, such as demographic data, and make appointments and order repeat prescriptions, but they will not be able to see the young person's past appointments or clinical record.
