6. Fill in the estimated loss details for each category, including those for structures affected, ensuring to mark the number of those minor, major, destroyed, uninhabitable, and inaccessible.
7. Move to the public assistance section, selecting the category of work applicable. Enter the estimated cost for each category and specify if work is insured, if the responsibility lies with another federal agency, and provide descriptions of the impact in each relevant section.
8. Add any additional comments or necessary information in the comments section. If more space is needed, be prepared to include a continuation page.
9. Once you have completed the form, save the document to your computer. Review all entries for accuracy before final submission.
