9. Attach a copy of the organization’s 501(c)(3) determination letter and, if applicable, a signed W-9 form. Note that the request will not be considered without these documents.
10. Select one funding category that best fits your project: Youth/Education, Community Outreach, Elders, Youth/Sports, Adult/Sports, or Other.
11. Write a brief summary of your program and the reason for your grant request, keeping it within 50 words.
12. Describe how your program will benefit the Lummi Nation and the surrounding area in a few sentences.
13. Explain your organization’s mission, the community it serves, and indicate whether the staff is comprised of volunteers or paid personnel.
14. If applicable, mention whether you expect funds from other sources to support the project and identify these sources.
15. Review the entire application for accuracy. Save any changes and consider downloading or printing your application for your records.
