2. Enter the School Contact Information, which includes the school name and campus, street address, city, state, ZIP code, phone number, and website URL. Ensure all entries are correct and formatted properly.
3. Fill out the Student Information section by entering the student's full name as registered in DEERS, the school-issued student ID (or N/A if applicable), the program or degree name, type, duration, scheduled start date, estimated completion date, and course delivery format.
4. Provide a detailed Program/Degree Overview that describes the program, including specific information required by the guidance.
5. Complete the Degree/Certification/Licensure Eligibility section by stating the name of the degree or certification the student will receive upon completion of the program.
6. Detail the Tuition Cost and Student Cost, which should include total tuition costs, student costs for materials, and the overall total. Remember, the MyCAA Scholarship covers up to $4,000 for tuition only.
7. List the Course Breakdown by entering course or program code, title, and credits (if applicable) for each course for which MyCAA financial assistance is being requested. Add rows as necessary to accommodate all required coursework.
8. Have the School Official Certification section completed by an authorized school representative, who will sign to certify that the information is accurate and true.
9. Review all entered information for accuracy. Once complete, save the document in PDF format for submission.
