- If necessary, provide the intermediary bank details, including the bank key or routing number, financial institution, and BIC (SWIFT code). Note that this is required for payments made in a currency that is not the vendor's local currency.
- If available, fill in the DFAT contact details with the contact name and phone number.
- For vendor authorization, print the name of the person authorized to submit the information, along with their phone number and a signature (note that a signature is not required if submitting via email).
- Once all fields are completed, save your changes, and decide whether to download, print, or share the form. Submit the completed form to the DFAT Accounts Unit via email or fax.
