Government Code Section 62.109 allows for a permanent or temporary exemption from jury service based upon a physical or mental impairment. It is impossible, or very difficult, for me to serve on a jury because of a physical or mental impairment. The following affidavit for excuse from Jury Service. A person requesting an exemption due to a physical or mental impairment must attach to the affidavit a statement from a physician. Please submit the request and completed physician's statement to the Jury Administrator at 112 West Beauregard, San Angelo, Tx 76903. Of jury service schedule changes. The period of time for which a temporary exemption is requested. This exemption requires a statement from your doctor explaining your condition and why you are unable to serve jury duty. If you have a physical or mental impairment you may request a permanent exemption from jury service.
