- In the New Student Enrollment Fee Information, note the enclosed fee and check number if applicable. Ensure you have included a separate form for each new student.
- Finally, both guardians must sign and date the form, confirming compliance with school regulations. Once completed, submit the form and any required fees to the ACS Administrative Offices as indicated.
- After completing the form, you can save the changes, download a copy for your records, print for physical submission, or share as needed.
