What is a 1004 Appraisal Report? A 1004 appraisal form is considered to be one of the most commonly used forms in residential real estate appraisals. In ance with USPAP a 1004, or URAR, is completed when estimating the market value of a single-family dwelling complete with an exterior and interior inspection.

An Appraisal Update and/or Completion Report (Fannie Mae Form 1004D) is used to update the value of an appraisal that is more than four months old or to confirm completion of, construction of, or renovations to a dwelling when the original appraisal was performed prior to completion.

For a detached unit in a condo project that includes a mixture of both attached units and detached units, the condo appraisal form Form 1073 must be used even when the subject unit is a detached unit. The appraiser may only use Form 1004 for a detached condo unit when the project consists solely of detached units.

The Small Residential Income Property Appraisal Report (FHLMC 72/FNMA 1025) is for the appraisal of two- to four-unit properties, including properties in PUD, condo or co-op projects.

An Individual Condominium Unit Appraisal Report (ICUAR) is for appraisals of one-unit properties in condo projects based on interior and exterior property inspections. Also known as Fannie Mae Form 1073, appraisals reported on this form must be completed in ance with the UAD Specification.

A URAR form, also known as Fannie Mae Form 1004, contains information needed to complete a full appraisal of a property using three primary approaches to determine value: cost approach, sales comparison approach and income approach.
