9. In the 'Customer Declaration' section, read and acknowledge the 'Important Notes'. If you authorize HSBC Insurance to update their records, tick the applicable box.
10. Sign the form as the principal account holder and date it appropriately. If there is a joint account holder, ensure they also sign and date the form.
11. After filling out all necessary sections, save your changes. You may then download, print, or share the form as required.
