Follow the steps to fill out the donation request form with ease.

1. Press the ‘Get Form’ button to obtain the form and open it for editing.
2. Begin by entering the date in the designated field at the top of the form.
3. Select the type of organization from the 'Org Type' section by checking the appropriate option.
4. Fill in the name of your event in the 'Event Name' field.
5. Specify the date of the event in the 'Event Date' field.
6. Provide your organization's name in the 'Organization' field.
7. Enter your complete address, including city, state, and zip code in the respective fields.
8. Indicate whom the event will benefit in the 'Whom does this event benefit?' section.
9. Detail how the Center will be acknowledged in the designated field.
10. Input your phone number in the 'Phone' field.
11. Complete the 'Contact Name' and 'Contact E-mail' fields with the relevant information.
12. Indicate whether your organization has received donations from the Center in the past.
13. If applicable, describe how the Center was acknowledged for previous donations.
14. Provide a detailed description of your donation request in the 'Donation Request Description' section.
15. Use the 'Notes' section for any additional information you want to convey.
16. Once you have completed the form, save it to your computer and decide how you wish to submit it — either via email to Nancy Deel at nancyd@alaskasealife.org or by mail to the address provided in the form.
17. You may also choose to print or share the form as needed.
