4. Fill out the home address of the visitor, along with the name of the parent or guardian and their home phone number.
5. Select the reason for the visit by checking one of the options: ½ Day AM or ½ Day PM.
6. Ensure the parent or guardian provides their signature and the date where indicated.
7. The form will also require an administrator’s signature and the date upon completion.
