- Detail the Cash Amount to Report/Remit, and any Additions or Deductions that apply to the original amount. Ensure you record the final total in the Cash to Report/Remit field.
- If applicable, input the Interest Rate, CUSIP Number, Current Shares, Original Shares, and Certificate Number.
- Provide the Registered To name, Issue Name, Sub-Issue Name, and Symbol for securities reported.
- Fill in details related to tangible properties, such as Tangible Box Number, Opened By name, Date Opened, Lease Expire Date, Unpaid Rent, and Drilling Fees where relevant.
- Enter the mandatory Social Security Number/FEIN for the property owner along with their Date of Birth in MM/DD/YYYY format.
- Complete the Owner Type and Relationship Type fields, entering pertinent information for the owner’s name, including first, middle, last name, prefix, suffix, and any applicable titles.
- Finally, confirm the Owner's last known address, including Street Address, Zip Code, City, State, County, and Country, ensuring all fields are accurately populated.
- Once all sections are complete, you can save changes, download, print, or share the form as necessary.
