  6. Enter the new tire fees due on Line 2 based on the number of new tires sold during the reporting period, assessed at $1 per tire.
  7. Document the total lead-acid battery fees due on Line 3, calculated at $1.50 per new or remanufactured battery sold.
  8. Report the rental car surcharge due on Line 4 for the rental of any for-hire passenger motor vehicles by total amount due from Form DR-15SWS.
  9. Sum the amounts on Lines 1 through 4 and enter the total on Line 5 on the back of the return, also bringing this total to Line 5 on the front of the return.
