9. Sign the affirmation statement confirming the truthfulness of your application, along with the date. If under 18, a parent or guardian must also provide a signature.
10. Complete the additional required forms, including a current driving record, criminal background record, I-9, W-4 forms, copies of your driver’s license, Social Security card, certifications, immunizations, and photo release form.
11. Once all sections are filled out completely, review your application for accuracy. Save any changes made, and prepare the document for submission.
