  10. In Line 9, indicate whether you are currently registered with the Connecticut Department of Revenue Services (DRS). If yes, enter your Connecticut Tax Registration Number in the upper right corner.
  11. Provide a detailed description of your business operations in Line 10.
  12. List any lessors who lease vehicles to you in Line 11, attaching additional sheets if necessary.
  13. Complete Line 12 by entering the total number of motor vehicles requiring decals. Multiply this number by $10 to calculate the amount due.
