Form 151a is used to waive the right to the survivor benefits that are payable to your retirement-date spouse if you die after your pension start date. (The surviving spouse pension is called a "joint and survivor" pension on the waiver form.) The content of Section 3 on the Form 151a is required by the PBA.

If you terminate your employment with less than two years of pensionable service, a return of contributions is your only pension benefit entitlement under the public service pension plan. A return of contributions is a lump sum equal to the pension contributions you paid into the plan, plus accrued interest.

You can request an emergency salary advance (ESA) when you are not paid. You must ask the Public Service Pay Centre to send your department a requisition (Form GC 80) requesting an emergency salary advances payment of estimated net salary.

You'll receive a T4A(P): Statement of Canada Pension Plan Benefits slip if, during the year, you received benefits from either the Canada Pension Plan (CPP) or the Québec Pension Plan (QPP).
