- In the Receipt of Electronic Notices section, choose Yes or No based on your preference to receive information electronically instead of via paper mail, and provide a valid work email address for receiving notifications.
- In Section B, appoint a Nominated Authority by entering their title, name, job title, contact numbers, and work email address for further communication.
- Ensure to select a preferred day and time for NAB to call your Nominated Authority.
- Review all the entries made and ensure all necessary signatures are filled out in Section C, along with the date.
- Double-check that you have completed any additional information required in Sections D and E, if applicable.
- After completing the form, you may save your changes, download a copy, print it, or share it as needed.
