- If choosing a self-managed super fund (SMSF) in section 4, complete the required fields including fund ABN, fund name, and attach the documentation confirming that your SMSF is an ATO regulated super fund. If applicable, make the declaration that you are the trustee or a director of the corporate trustee.
- In section 5, provide your signature and the date, ensuring that you have attached all relevant documents as required.
- Return the completed form to your employer as soon as possible for further processing.
