The Lausd Form Hi 22 is a two-page form that collects employee information for the Los Angeles Unified School District. This form is used to determine eligibility for employment with the school district, and it must be completed by all applicants.

Benefits under public pension plans are typically based on a combination of age and years of service. Under normal retirement eligibility requirements, a Los Angeles teacher qualifies for full pension benefits at age 62 with five years of service.

If your retirement formula is 2% at 62, for example, this means you get 2% of your pay if you retire at age 62 . Age 62 is referred to as your “normal retirement age .” Local miscellaneous members receive one of six retirement formulas, with varying retirement ages and final compensation percentages .

To qualify for lifetime benefits: Qualifying years in paid service: A year of paid service means you were paid for at least 100 full-time days and you were eligible for District benefits. 2. Plan enrollment at retirement: On the date you retire, you must be enrolled in the plans you wish to continue during retirement.

For employees hired on or after June 1, 1992, years of qualifying service and age must total at least eighty (80) in order to qualify for retiree health benefits. For employees who have a break in service, this must include at least ten (10) consecutive years immediately prior to retirement.

Retirement Calculation Formula Your retirement benefit is calculated using a formula with three factors: Service credit (Years) multiplied by your benefit factor (percentage per year) multiplied by your final monthly compensation equals your unmodified allowance.
