- Begin by entering the dependent student’s information in section A. This includes the student’s last name, first name, middle initial, permanent street address (with apartment number, if applicable), city, state, zip code, date of birth, home phone number, email address, and alternate or cell phone number.
- In section B, provide details about the family household. List yourself, your parent(s) and stepparent, any other children your parent(s) support, and other individuals living with them who receive support. Also, indicate the college names of any siblings who will be enrolled at least half-time during the relevant academic year.
- Proceed to section C for tax return filers. If you or your parent(s) filed a tax return and did not use the IRS data retrieval, attach a copy of the 2017 federal tax return transcript or signed tax return.
- In section D, for tax return non-filers, if you or your parent did not file taxes and were not required to, submit an IRS Verification of Non-Filing Letter. If obtaining this letter is not possible, include a signed NJCU Non-Filing statement.
- Section E addresses student income. Indicate whether you used the IRS Data Retrieval Tool, state your employment status, and detail any 2017 income, including employer names and amounts earned.
- Proceed to section F for parent income. Similar to section E, indicate if your parent(s) used the IRS Data Retrieval Tool, state their employment status, and provide details of their income.
- Finally, complete the certifications and signatures section. The student and one parent must sign and date the form to certify that all reported information is complete and accurate.
