11. Select your preferred direct debit option, either from a bank account or credit/debit card, and complete the corresponding fields.
12. If selecting a bank withdrawal, provide the Name of Bank, Routing Number, and Account Number, indicating whether it is a checking or savings account.
13. If choosing to debit from a card, fill in the Type of Card, Name of Cardholder, Address of Cardholder, Card Number, and Card Expiration.
14. If you prefer to receive monthly invoices via email, check the appropriate box to indicate your preference.
15. Finally, review all the information for accuracy, then save the changes, download, print, or share the completed form as needed.
