- Fill in the date of removal from service, specifying the reason such as incapacitation, superannuation, or dismissal. Next, detail the employee's pay structure by entering the basic pay, special pay, technical pay, personal pay, and qualification pay.
- Continue by noting the audit office and the audit office personal number. Additionally, indicate any periods of unpaid contributions to the Benevolent and Group Insurance Funds.
- In Part III, provide details of the prospective beneficiary. Record their name, CNIC, date of birth, relationship to the deceased, profession, marital status, and contact information.
- Next, fill out the nearest National Bank of Pakistan branch details to the beneficiary's residence.
- For the certificates required by the authorized officer, ensure the officer certifies the accuracy of the information and that the employee was a regular contributor to the funds.
- Complete the form by providing the necessary signatures and dates where indicated, ensuring all information is correct and all necessary documents are attached.
- After reviewing all fields for accuracy, users can save their changes, download the completed form, or print and share it as needed.
