14. Provide your National Tax Number and Sales Tax Registration Number.
15. Enter the date of incorporation and commencement of commercial production, and indicate the total number of employees.
16. State whether your business is a proprietorship, firm, or company, and indicate the status (public or private).
17. Choose the class of membership you are applying for (Associate or Corporate).
