- Specify any data exchange agreements in place for sharing PII with other systems or agencies.
- Answer whether individuals can object to or consent to the collection of their PII and describe the methods accordingly.
- Indicate what information is provided to individuals about the collection of their PII.
- Review the completed form for accuracy, save any changes made, and choose options to download, print, or share the form as needed.
