7. Select the type of application by ticking the relevant boxes, such as new supply, temporary supply, or upgrading of supply.
8. Indicate the type of development where the installation will take place, selecting from options like landlord’s installation, industrial, or residential.
9. Specify the metering scheme type and provide a brief description of your business and total supply capacity.
10. In Part III, detail the quantities of various electrical components you are applying for, such as socket outlets and other equipment. Ensure that all entries are accurate as amendments are not allowed for HDB residential units.
11. Ensure the licensed electrical worker (LEW) fills in their name and signature at the end of Part III along with their license number.
12. If applicable, ensure that you obtain an endorsement from the Housing & Development Board (HDB) as detailed in Part IV.
13. After completing the form, review all entries for accuracy, then save your changes, download, print, or share the form as needed.
