  9. State whether the property was constructed, purchased, acquired, or adapted by selecting the appropriate option.
  10. Enter the chargeable value of the property as of May 1, 2013.
  11. Provide the date when the property was constructed, purchased, acquired, or adapted.
  12. If adaptation work has been completed, describe the nature of this work.
  13. Specify the cost associated with the adaptation work.
  14. Estimate the chargeable value of the property prior to the adaptation work.
  15. Input the date that the adaptation work was completed.
  16. Read the declaration section. Confirm that all particulars on the form are accurate to the best of your knowledge.
  17. Sign the declaration, including your phone number and email address.
  18. Save your changes, and download or print the completed form for your records.
  19. Submit your completed application form to the Revenue Commissioners, LPT Branch, PO Box 1, Limerick.
