Follow the steps to complete the P1 Form effectively.

- Enter your corporate name and mailing address in the designated fields. If your mailing address has changed, line out the old address and print the new one to the right.
- If the principal office address differs from the mailing address, provide that address, including the city, state, and ZIP code.

- Specify the nature of your business. If inactive during the reporting period, state 'Inactive.' Corrections should be lined out and printed below this section.
